Access Your 2025 1095-C Today

In accordance with the Affordable Care Act (ACA) employer reporting requirements, Form 1095-C for the 2025 tax year is available to you upon request.
The University has made Form 1095-C accessible electronically through the employee
dashboard.
You may access and download your form by following the instructions below:
1. Log in to MyISU.
2. Go to the Time Entry & Employment Details card.
3. Click on Employee Dashboard and login.
4. Click on Taxes.
5. Select 1095-C Employer Provided Health Insurance Offer and Coverage
Statement.
6. Choose 2025 from the Tax Year drop down menu.
7. Click on Display.
If you prefer to receive a paper copy of your Form 1095-C, you may request one by contacting the payroll department at payroll@isu.edu or (208) 282-1353. A paper copy will be furnished within 30 days of receiving your request.
